Cosa Nostra is a mod that dates back to 2011 as a Wolf4SDL mod. Years before the shift to Wolf3D it started out as a Doom mod and eventually turned into a ECWolf project down the road. The total conversion is supposed to take place in the year 1940 featuring a mafia theme around Chicago. As of now not much is known about it and if it's ever gonna be released.
